FORM AND ENDORSEMENT CHANGES

COUNTRYWIDE

Commercial Automobile

ISO has introduced the following forms:

CA 23 37 01 02 - War, Military Action And Terrorism Exclusion

CA 23 38 01 02 - War, Military Action And Terrorism Exclusion - Garage Coverage Form

CA 26 14 01 02 - Amendment Of Single Interest Policy Provisions - War, Military Action And Terrorism Exclusion

These forms replace the policy's previous war exclusion language to create, essentially, an absolute exclusion against damage that is caused by military or terrorist actions. There is also a newly defined term, "terrorism."

What follows is a list of individual states and which of the above changes each has approved involving ISO forms and programs. The text will not be repeated but will refer back to this Countrywide section.

Changes Made by Individual States
ALABAMA

Commercial Auto--The program changes referred to in the Country-wide section have been approved for use by all participating insurers. The forms are available for all interested insurers with a distribution date of February 20, 2002.

COLORADO

Dwelling Liability, Dwelling Program, Homeowners--The optional exclusions filed by ISO to exclude coverage for terrorism have been approved for use. The following forms are available for all interested insurers with a distribution date of January 1, 2002:

* DL 24 08 01 02 Exclusion of War And Terrorism

* DL 98 29 01 02 Personal Umbrella Liability Policy War and Terrorism Exclusion Endorsement

* DP 05 36 01 02 Exclusion of War And Terrorism

* HO 05 17 01 02 Exclusion of War And Terrorism

Personal Auto--Form PP 04 25 07 02, Uninsured Motorists Coverage - Colorado has been approved for use by all participating insurers. The form is available for all interested insurers with a distribution date of July 2002. The form was revised to delete the "owned-but-not-insured" exclusion as a result of a recent Colorado Supreme Court Decision.

DELAWARE

Homeowners--The ISO optional homeowner endorsements for excluding damage caused by rot or mold have been approved. The following forms are available for use by all participating insurers
for all policies written on or after July 1, 2002:

* HO 04 26 04 02 Limited Fungi, Wet or Dry Rot, or Bacteria Coverage

* HO 04 27 04 02 Limited Fungi, Wet or Dry Rot, or Bacteria Coverage

* HO 04 28 04 02 Limited Fungi, Wet or Dry Rot, or Bacteria Coverage

DISTRICT OF COLUMBIA

Commercial Auto--The program changes referred to in the Countrywide section have been approved for use by all participating insurers for all policies written on or after
March 15, 2002.

GUAM

General Liability--ISO's multistate program revision introducing the CG 04 37 Electronic Data Liability Endorsement and editorial clarifications to several forms has been approved. The forms are available for use by interested insurers with a distribution date of August 2002.

IDAHO

Dwelling Liability, Homeowners--ISO's optional exclusions for loss caused by terrorist acts have been approved for use under special circumstances. A company which can document that its solvency could be threatened by its exposure to terrorist-related loss may be allowed to file the exclusions, using the language found in the ISO forms. Interested companies must make their own filing.

INDIANA

Homeowners--Form HO 01 13 06 02, Special Provisions - Indiana has been approved for use by all participating insurers for all policies written on or after June 1, 2002. The form was changed so that its cancellation provision conforms with the requirements of former House Bill 1555.

MAINE

Commercial Auto--The program changes referred to in the Countrywide section have been approved for use by all participating insurers for all policies written on or after March 19, 2002.

Personal Auto--Form PP 13 01 12 99, Coverage For Damage To Your Auto Exclusion Endorsement has been approved for use by all participating insurers for all policies written on or after July 1, 2002. This form is intended to clarify that a PAP does not obligate an insurer to consider any diminished value of a car that has been repaired after a covered accident.

MARYLAND

Businessowners--ISO's multistate forms filing introducing their 2002 BOP program, along with the following state-specific forms, have been approved for use by all participating insurers for all policies written on or after October 1, 2002:

* BP 01 64 07 02 Maryland Changes

* BP 04 44 07 02 Maryland Changes - Liability For Hazards of Lead

* BP 17 14 07 02 Maryland Condominium Additional Provisions

Commercial Lines--ISO's multistate form filing introducing form IL 09 35 07 01, Exclusion of Certain Computer-Related Losses endorsement has been approved for use by all participating insurers for all policies written on or after July 1, 2002.

MINNESOTA

Commercial Auto--The program changes referred to in the Countrywide section have been approved for use by all participating insurers for all policies written on or after January 1, 2002.

MISSISSIPPI

Dwelling Liability--Form DL 24 71 04 02, Limited Fungi, Wet or Dry Rot, or Bacteria Coverage has been approved for use by all participating insurers for all policies written on or after July 1, 2002. This form provides limited coverage for loss suffered by a third party which involves wet or dry rot or bacteria and for which an insured is liable.

Homeowners--The ISO optional homeowner endorsements for excluding damage caused by rot or mold have been approved. The following forms are available for use by all participating insurers
for all policies written on or after July 1, 2002:

* HO 04 27 04 02 Limited Fungi, Wet or Dry Rot, or Bacteria Coverage

MISSOURI

Commercial Auto--The program changes referred to in the Countrywide section have been approved for use by all participating insurers for all policies written on or after February 28, 2002.

NEVADA

Businessowners--ISO's multistate forms filing introducing their 2002 BOP program, along with state-specific form BP 01 88 07 02 Nevada Changes, have been approved for use by all participating insurers for all policies written on or after August 1, 2002.

Personal Auto--Form PP 01 82 07 02, Amendment of Policy Provisions - Nevada has been approved for use by all participating insurers for all policies written on or after July 1, 2002. The form was changed to delete a reference to a previous requirement that certain disputed losses be submitted
to arbitration.

NEW HAMPSHIRE

Commercial Auto--The program changes referred to in the Countrywide section have been approved for use by all participating insurers for all policies written on or after March 29, 2002.

NEW MEXICO

Commercial Auto--The program changes referred to in the Countrywide section have been approved for use by all participating insurers for all policies written on or after March 14, 2002.

NEW YORK

Homeowners--ISO's multistate forms filing introducing their Homeowners 2000 program, along with a number of state-specific forms, have been approved for use by all participating insurers for
all policies written on or after October 1, 2002.

NORTH CAROLINA

Commercial Lines--The ISO multistate introduction of interline forms for facilitating the use of their Farm Liability Umbrella policy has been approved for use by all participating insurers for all policies written on or after September 1, 2002.

Farm--The ISO multistate filing introducing their Farm Liability Umbrella policy and endorsements has been approved for use by all participating insurers for all policies written on or after September 1, 2002. The umbrella form may be used in conjunction with an underlying standard farm and auto (personal or commercial) policy.

NORTH DAKOTA

Commercial Auto--The program changes referred to in the Countrywide section have been approved for use by all participating insurers for all policies written on or after January 18, 2002.

Dwelling Liability, Dwelling Program, Homeowners--The optional exclusions filed by ISO to exclude coverage for terrorism have been approved for use. The following forms are available for all interested insurers with a distribution date of January 1, 2002:

* DL 98 29 01 02 Personal Umbrella Liability Policy War and Terrorism Exclusion Endorsement

OREGON

Dwelling Liability--Form DL 24 71 04 02, Limited Fungi, Wet or Dry Rot, or Bacteria Coverage has been approved for use by all participating insurers for all policies written on or after July 1, 2002. This form provides limited coverage for loss suffered by a third party which involves wet or dry rot or bacteria and for which an insured is liable.

TENNESSEE

Homeowners--The ISO optional homeowner endorsements for excluding damage caused by rot or mold have been approved. The following forms are available for use by all participating insurers for all policies written on or after May 1, 2002:

HO 04 32 05 02 Limited Fungi, Wet or Dry Rot, or Bacteria Coverage

UTAH

Commercial Auto--The program changes referred to in the Countrywide section have been approved for use by all participating insurers for all policies written on or after January 23, 2002.

VERMONT

General Liability--ISO's multistate program revision introducing the CG 04 37 Electronic Data Liability Endorsement and editorial clarifications to several forms has been approved. The forms are available for use by interested insurers for all policies written on or after August 1, 2002.

VIRGIN ISLANDS

Businessowners--ISO's multistate forms filing introducing their 2002 BOP program has been approved for use by all participating insurers for all policies written on or after August 1, 2002.

WASHINGTON

Commercial Liability Umbrella, General Liability--The following forms have been approved for use by all participating insurers for all policies written on or after August 1, 2002:

* CU 26 77 05 02 Washington - Fungi or Bacteria Exclusion

* CG 26 25 05 02 Washington - Fungi or Bacteria Exclusion

* CG 26 78 05 02 Washington - Limited Fungi or Bacteria Exclusion

* CG 31 49 05 02 Washington - Fungi or Bacteria Exclusion

* CG 31 50 05 02 Washington - Limited Fungi or Bacteria Exclusion

These forms permit either the complete or partial exclusion of coverage for damage caused by mold and similar sources. They were modified from the multi-state versions to eliminate language regarding concurrent causation.

WEST VIRGINIA

Commercial Auto--Form CA 01 83 07 02, West Virginia Changes has been approved for use by all participating insurers for all policies written on or after July 1, 2002. The form was changed to eliminate a reference to the inapplicability of the ISO multistate Diminution of Value exclusion from the form's section on physical damage.

WISCONSIN

Farm--The ISO multi-state filing introducing their Farm Liability Umbrella policy and the following state-specific forms have been approved for use by all participating insurers for all policies written on or after September 1, 2002:

* FB 01 29 07 02 Wisconsin Changes

* FB 04 99 07 02 Wisconsin Changes - Farm Premises And Personal Umbrella Liability

The umbrella form may be used in conjunction with an underlying standard farm and auto (personal or commercial) policy.

WYOMING

Dwelling Liability--Form DL 24 71 04 02, Limited Fungi, Wet or Dry Rot, or Bacteria Coverage has been approved for use by all participating insurers for all policies written on or after June 1, 2002. This form provides limited coverage for loss suffered by a third party which involves wet or dry rot or bacteria and for which an insured is liable.
